The Inpatient Care Coordinators are the inpatient case managers that work directly with patients, family, nursing staff, and providers in providing necessary support to the patient and planning for discharge from our facility.  The IPCCs work together with the Social Services Clinicians and multidisciplinary team in planning a safe discharge, identify any assistive equipment that will be needed (i.e. walkers, transfer benches, etc), identify any medical needs, works with other health care agencies and organizations to facilitate placement needs and discharges to said facilities,  handles follow-up appointment requests from providers within our facility and to other health care organizations, handles referrals to other health care agencies; completes a discharge assessment on every patient admission, and receives and reviews swing bed referrals to our facility.  The IPCCs also support Utilization Review (UR) and review charts every morning to ensure patients meet continued stay criteria to satisfy federal regulations. This is a great role for a RN with Case Management or Utilization Review Experience!   Salary: $44.78-$57.28 per hour, DOE.  $25,000 Sign on Bonus, $10,000 Relocation Bonus

This position works intensively with advanced systems to track and trend denials, build models for documentation improvement plans, and coordinate with all areas and levels of the hospital. This position works closely with the provider community to improve clinical documentation, and with administration to ensure appropriate utilization of hospital resources.   Under general supervision, provides professional assessment, planning, coordination, implementation, and reporting of complex clinical data. Maintains compliance with regulations affecting utilization management. Provider communication and behavior change can be challenging; this individual must be a master communicator and change agent. Clinical background required, as this position will be required to read, rapidly synthesize, and narrate a clinical event to patients’ third-party coverage for accurate and timely authorization and reimbursement.
