This position will be based in one of the SEARHC Rehabilitation Services settings (hospital, outpatient clinic or home health) and will have a varied focus depending on the location and the local need.  The primary focus will be to provide general Physical Therapy (PT) evaluations and treatments which impact patient’s functional abilities.  Physical therapy is a core service provided by this healthcare organization.  The therapist performs in a professional manner, respects confidentiality, and demonstrates qualities endorsed by following the Seven Standards for Service Excellence outlined in the SEARHC Human Resources Manual.   Services are to be provided in compliance with hospital standards, licensure requirements, regulatory and practice standards of Physical Therapy and the national professional guidelines established by the American Physical Therapy Association (APTA).   Physical therapists are health care professionals responsible and accountable for patient assessment, evaluation, establishing a plan of care and theraputic interventions for a wide range of ages with widely varying diagnoses and skills needs who present with a full spectrum of movement abilities, dysfunctions and complexities.  The clinician needs to be able to inform clinical teams regarding decisions that involve physical therapy while taking into consideration consultation provided by other care providers. As patients are seen, recommendations may be made regarding their level of independence and rehabilitation needs which may impact services, care placement and further treatment.  The incumbent shall also demonstrate accountability for the contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion.  Documenting standards must be followed for continuity of care and reimbursement for services. The position may require occasional travel off site and out of town to provide services or cover for providers in other SEARHC facilities.   Salary - $43.77-$53.45/hour DOE

This position is available for after-hours telephone contact for clients in crisis when needed and as assigned; in addition to providing clinical support in person or by phone as needed at any SEARHC location within the Behavioral Health Division.   There is no specific responsibility to managing an operating budget.  As a clinician the financial responsibilities lie in documenting all services as outlined first and foremost in the code of ethics of the profession. Clinicians maintain records necessary for rendering professional services to their clients and as required by laws, regulations, or agency or institution procedures. Clinicians include sufficient and timely documentation in their client records to facilitate the delivery and continuity of needed services. Clinicians take reasonable steps to ensure that documentation in records accurately reflects client progress and services provided. If errors are made in client records, clinicians take steps to properly note the correction of such errors according to agency or institutional policies, proper documentation is a requirement to ensure that SEARHC is able to bill accurately for services provided to client in accordance with state, federal, and insurance rules, laws and regulations.   This position is a covered position in accordance with the Indian Child Protection Act (ICPA).   Salary Range: Unlicensed: $33.41-$44.19   Licensed: $35.72-$47.38

This position also provides clinical services such as intake/diagnostic evaluations, case evaluations, and crisis intervention; as well as acts as a liaison with other agencies as required. This position is a member of an interdisciplinary treatment team and provides regular clinical supervision, ensures proper maintenance of clinical records in accordance with insurance regulations and established SEARHC policies and procedures. A clinician ensures that clinical productivity standards as established and followed. This position is available for after-hours in person Crisis Interventions for clients through our Hospital Emergency Room or Police Department or by phone as needed at any SEARHC location within the Behavioral Health Division.    Clinicians who are not yet licensed will receive clinical supervision that meets the requirements stated under the specific license that one is becoming licensed in with documentation of the supervision kept on each person. Clinicians not licensed will provide structured chemical dependency and mental health services with a focus in supervision on providing the appropriate delivery of twelve core functions:  Screening, intake, orientation, assessment, treatment planning, therapy, case management, crisis intervention, client education, referral, reports and records keeping, and consultation.

POSITION SUMMARY This position functions within the SEARHC Behavioral Health department to provide substance use treatment services for the SEARHC Intensive Substance Abuse and Addiction (ISAAT) program. The Chemical Dependency Counselor (CDC) role includes provision of the following services: screening, assessment, treatment planning, referral management, counseling, crisis support, case management and other recovery supportive services. ISAAT services are provided through individual, group, and family services, including telehealth platforms.   POSITION COMPLEXITIES A CDC position requires a thorough understanding of substance abuse and dependency, as well as a familiarity and aptitude in implementation of evidence-based interventions and treatment strategies for effective disease management. The CDC provider responsibilities include screening, assessment, treatment planning, referral management, counseling, crisis support, case management and other recovery supportive services. This role also requires the aptitude, and/or the ability to develop the skills required, to complete necessary oral and written communication and documentation in the electronic health record. Position responsibilities include a responsibility to provide consultation to assist other staff members throughout the consortium in substance abuse and dependency related care needs.      CDC I Salary Range: $27.12 – 35.54/hr. CDC II Salary Range: $31.11 – $40.99/hr.

CA III must process provider orders pertaining to diagnostic tests, referrals and other patient data that involves non-routine problems, timely and accurately.     The Clinical Assistant III provides care to infants, toddlers, children, adolescents, adults and the elderly in an ambulatory care clinic.  This position is essential in maintaining efficient clinic operations and patient flow in order to maximize the provider’s time and to insure access to care.  Clinical Assistants perform administrative duties, data management and handle basic clinical tasks.  This information is recorded in patients’ chart along with patient’s medical history.  This position works closely with providers giving support such as rooming patients, performing Point of Care Testing (POCT), performing lab draws, and provide immunizations and other injections within the scope of practice certification, ECC monitoring, answering phones, processing provider orders data entry and participation in training processes.  Must be able to make independent decisions within scope of practice based on experience and good judgment and be self-starting and require little guidance to keep busy.   No travel, full-time 40 hours per week.  No weekends or nights.

There is a risk of constant exposure to illness, injury, and disease. There are sometimes large volumes of patients within a short period of time. The Clinical Assistant must be able to multitask and prioritize duties, as the pace is often rapid with pressure to efficiently complete assignments and keep clinic flow moving well. The Clinical Assistant must be able to recognize signs and symptoms that may require immediate attention. The duties of the Clinical Assistant III require more critical judgment in complex and variable situations. Requires attention to detail in order to accurately collect patient data, complete patient documentation, maintain logs, and comply with regulatory requirements. Required to work with staff and patients of different backgrounds and educational levels and the CA must be able to communicate clearly. The person in this position must be able to provide compassionate care to individuals in various states of health.   Pay range $26.06 to $28.43 per hour, DOE.
