Job Overview:

 

We are currently looking for a Advanced Community Health Aide or Community Health Practitioner for our Multiple locations in Southeast Alaska.

Salary - $100,000-$103,000 DOE.

Responsibilities:

Key Essential Functions and Accountabilities of the Job

  • Provides clinical services as a primary health care provider, including acute care, WCC, return prenatal, chronic care, preventive and wellness services, using the CHAM to obtain patient problem-specific histories, performs physical exams and lab work, makes assessments independently (with signed Standing Orders) or in consultation with the reporting provider, and implementing the CHAM plan for the assessment.
  • Provides medications as directed by the reporting provider and/or the CHAM.
  • Performs tasks approved as standing orders at a higher level of complexity, skill, and knowledge than those listed in the CHAM when approved in writing by the medical supervisor.
    • Provides other patient care assistance to providers as directed.
  • Assists with clinic administrative tasks, including entering findings and data in patient charts, QA/QI tests relating to laboratory tests, packages and ships laboratory specimens, cleaning and sterilizing instruments, ordering clinic supplies, maintaining an inventory and ordering pharmaceuticals as needed, checking for outdated supplies and medications.

 

Other Functions

  • Provides shared on call and emergency care as assigned based on field clinic location and staffing levels
  • Other duties as assigned

 

Qualifications:

  • Successfully complete Pre-Session and Sessions I - IV and the Community Health Practitioner (CHP) Preceptorship; attain CHP Certification; attain CHP Credentialing.
  • Continuing education training to maintain clinical proficiency (48 hrs/2 yrs).
  • All required:  CHP Certification, CHP Credentialing, Current EMT-1 Certificate, and Current BLS Certificate.
