Job Overview:

The primary purpose of this position is to perform technical duties and provide clerical support in the health information management function of the department.  To initiate accurate records and maintain records in a confidential, secure, complete, and accessible manner for current and future patient care. To provide for appropriate disclosure of records to authorized users.  To service both internal and external customers ensuring patient safety and continuity of care. This is an entry level position and the first step on the Health Information Management career ladder.

Responsibilities:

  • Knows, understands, incorporates, and demonstrates the SEARHC Mission, Core Values, and Vision in behavioral practices, policies, and decisions.
  •  
  • Initiates chart pick-up from the facility (floors/Nursing areas) or Clinics, as appropriate:
    • Retrieves/receives records from the clinic(s) or hospital.
    • Scans paper records into the Electronic Health Record.
  •  
  • Scanning:
    • Makes sure the documentation that is retrieved from the clinic/hospital gets scanned into appropriate patients record.  
    • Identifies documentation that is not to be scanned and discards into the appropriate shred bin.
    • Prep's and prepares health information documentation for electronic viewing.
    • Scans all appropriate health record documentation into the Electronic Health Record.
  •  
  • Indexing
    • Index the documents scanned into the electronic health record and verify that the information scanned is being placed into the correct patient, encounter, and folder.
    • Understands the Events Set Hierarchy Structure within the Electronic Health Record.
  • Importing
    • Checks for incoming email and faxes throughout the day
    • Ensures documentation received meets appropriate guidelines for patient identification and completion.
    • Demonstrates a knowledge of where and how to send back documentation that does not meet these criteria.
    • Appropriately imports documents to correct patient and patient encounter ensuring correct note type and tule is used.
  • Retrieving (Pulling)and Filing Paper Medical Records
    • File and retrieve medical records using the terminal digit filing system at locations that still support this function.
    • Perform routine file and shelf maintenance to keep files in order and to ensure they are accessible for patient care.
  • Clerical Support:
    • Performs needed clerical support within the HIM Department.   
  • Communication:
    • Respond courteously to telephone and personal requests by staff, patients, and other authorized users and provide health record needs or directs the requester to appropriate staff who can assist them.
    • Answer telephone calls and provide answers to basic questions regarding patient’s medical record (not clinical documentation) or request for medical records.
  • Release of Information Intake:
    • Understands a thorough working knowledge of HIPAA Privacy Practices and Release of Information policies and procedures and can assist patients and staff relative to HIM department responsibilities and authorizations.
    • Ability to route the patient, patient representative, or staff to the Privacy Officer, regarding patient access, requests for amendments, accounting of disclosures, patient portal, and/or any additional privacy related questions.
    • Evaluates basic release of information requests for appropriate disclosure of health records information according to consortium policy and procedure.
    • Able to determine if a Release of Information is valid, and follow the appropriate steps to log the Release of Information Request into the Electronic Health Record System.
    • Processes walk-in patients requesting medical records by providing guidance in the completion of the release of information request form and verifying identification.
    • Answers any calls or questions about the status of a ROI; look up the information within the Electronic Health Record tracking system and communicate the correct status.

Qualifications:

Education:  

  • Required High school or GED

Experience:

  • Computer applications course or 6 months experience with computer use.
  • 6 months clerical or customer service experience.  Prefer medical record or healthcare work experience

Other Qualifications:

  • Medical Terminology preferred 

Knowledge, Skills, and Abilities

Knowledge of:

  • Ability to learn the Electronic Health Record and components of the system
  • Ability to prioritize duties and route what you cannot perform to the correct person
  • Knowledge and ability to work in a data entry environment
  • Working knowledge of computers and computer keyboarding skills
  • Working knowledge of health record components

  • Ability to perform accurate and timely detailed work scanning a record into the EHR

  • Contribute to the overall success of the Health Information Management Department and the Southeast Alaska Regional Health Consortium
